  1. Cut along line 1.
  2. Cut along line 2.
  3. Cut along line 3.
  4. Fold panel A along line 4.
  5. Fold panel B along line 4, opposite side to panel A.
  6. Fold panel C onto panel E along line 5.
  7. Fold panel D onto panel E along line 6.
  8. Fold panel up along line 7, secure with paper clip.
